When the Community Care team began working together in 2017, little did they know the COVID-19 pandemic would soon become the stage for the critical work they are called to do. Frontline healthcare personnel have risked their lives for the care of patients in their communities. Physicians, scientists, researchers and others are focusing on the pattern of social determinants on patient outcomes and how to deliver high-quality, economically sustainable care.

In the video, Dr. Medina discusses leveraging data, making the right investments and engaging patients and the community for more optimal health outcomes.
