Dr. Myers has broad experience in population health as a model for improving healthcare quality while reducing costs.
Cleveland Clinic Community Care takes a new, population health approach to managing the care of patients. Rather than just simply addressing individual patients’ needs on a visit-by-visit basis, Community Care is leveraging a wealth of data and an expanded care team to proactively address the health needs of populations.
Cleveland Clinic clinicians and IT analysts have built customized registries and reports that enable providers to see the entire population of Cleveland Clinic patients, whether they’re coming in for appointments or not. Once groups of patients who have not recently seen a provider have been identified, any member of the patient care team — including physicians, advanced practice providers, nurses, care coordinators, medical assistants, pharmacists and social workers — can reach out to consenting patients individually to discuss their current health status and determine the appropriate care steps.
“We look forward to welcoming Dr. Myers to lead Cleveland Clinic Community Care, a model that was launched in January after a year of thoughtful development,” says Cleveland Clinic Chief of Staff Herb Wiedemann, MD. “While we already have made great strides in reorganizing our primary care practices to deliver on the promise of population health, which is to enhance health and value for the populations we serve, we are confident Dr. Myers will help us extend the model further.”
Dr. Myers will direct a team that includes physicians and advanced practice providers from Internal Medicine, Family Medicine, Hospital Medicine, Geriatrics and General Pediatrics, supported by Wellness, Express Care, Medical Care at Home, and the Quality Alliance so that Cleveland Clinic Community Care is best equipped to manage access, quality, experience, utilization and cost across the entire continuum of care and Cleveland Clinic network.
“I’m honored to join Cleveland Clinic in strengthening its legacy of service to the surrounding populations,” says Dr. Myers. “Cleveland Clinic Community Care will apply a data-driven comprehensive team approach to the clinical needs of patients; and partner with others, including community leaders, municipalities and other health systems, to address the social determinants of health impacting the populations in those communities. We also seek to influence the international policy dialogue around health system design and population health.”
Prior to coming to Cleveland Clinic, Dr. Myers served as senior vice president/Chief Medical Officer and operations officer of Texas Health Physicians Group/Enterprise; and chair of the Clinical Integration team at Southwestern Health Resources, a private not-for-profit clinically integrated network formed by University of Texas Southwestern Medical Center and Texas Health Resources. Previously, Dr. Myers was Chief Medical Officer and senior vice president for Methodist Health System. He has also served in several leadership positions with the American Hospital Association, most recently being selected to chair the Committee on Clinical Leadership for 2018 and 2019.
Dr. Myers earned his undergraduate degree from Centenary College of Louisiana in Shreveport, graduating from Louisiana State University Medical Center with honors. He completed his residency at Hillcrest Medical Center in Tulsa, Okla., and completed a fellowship in advanced obstetrics at the University of Oklahoma Obstetrics and Gynecology Department where he then taught for seven years. Additionally, he has a Masters in Health Care Management from the Harvard T.H. Chan School of Public Health.
Dr. Myers assumed this new leadership role with Cleveland Clinic Community Care on June 18.
