Combined hematology-gynecology/adolescent medicine clinic addresses major gap in care

Heavy menstrual bleeding can significantly disrupt daily functioning and quality of life for pediatric and adolescent female patients. While hormonal immaturity such as dysfunction of the hypothalamic-pituitary-ovarian (HPO) axis can contribute, up to one-quarter of these patients can also have an underlying bleeding disorder.

Cleveland Clinic Children’s launched a dedicated clinic to streamline evaluation and treatment for young patients presenting with heavy menstrual bleeding and iron deficiency anemia, including those with known or suspected bleeding disorders. Patients receive a comprehensive diagnostic evaluation, an individualized treatment plan for menstrual management and coordinated hematology-gynecology/adolescent medicine care within a single visit.
“Our goal is to provide multidisciplinary care by bringing two specialty visits together into a single appointment. This approach is valuable for families who travel long distances to reduce the burden of multiple visits,” says pediatric and adolescent gynecologist Erin Isaacson, MD.
Dr. Isaacson staffs the combined clinic alongside Veronica Issac, MD, an adolescent medicine specialist. Both physicians provide comprehensive menstrual management for a pediatric patient population.
Heavy menstrual bleeding lasting longer than seven days, bleeding through clothing or menstrual products, fatigue and iron deficiency anemia or acute anemia related to menstruation are all potential indications for an underlying bleeding disorder and warrant further workup.
“When these patients are referred to our clinic, we perform a comprehensive evaluation of both primary and secondary hemostasis to determine whether an inherited or acquired bleeding disorder is contributing to their symptoms,” says pediatric hematologist Marium Malik, MD, who launched the multidisciplinary clinic in 2025.

The diagnostic workup includes an assessment of platelet count and their function, von Willebrand factor, coagulation factor levels and other components of the coagulation cascade to identify abnormalities that may explain the excessive menstrual bleeding. Dr. Malik says patients as young as 11 or 12 have been diagnosed in the clinic, highlighting the importance of early recognition and evaluation.
Even with the absence of a bleeding disorder diagnosis, hematologic evaluation remains an essential component of care. All patients with heavy menstrual bleeding are assessed for iron deficiency anemia through measurement of iron, ferritin and hemoglobin levels, as chronic menstrual blood loss can significantly deplete stores.
“Longstanding iron deficiency can have consequences beyond anemia, affecting cognitive function, behavior and overall quality of life, while also contributing to symptoms of anxiety and depression,” adds Dr. Malik.
Hormonal management is a safe and effective therapy for heavy menstrual bleeding, with a wide variety of options. According to Dr. Isaacson, the best approach depends on the patient’s history, lifestyle, preferences and desired level of menstrual suppression.
Some patients with bleeding disorders require menstrual suppression at a young age to safely control bleeding, a recommendation that can understandably raise questions for the family. However, a combined visit with gynecology/adolescent medicine and hematology allows both specialists to discuss the rationale, benefits and safety of hormonal treatment options together.

“Our multidisciplinary approach allows us to clearly explain the risks of uncontrolled menstrual bleeding alongside the benefits and safety of available treatment options,” says Dr. Malik. “Having both specialists present during the same visit gives families the opportunity to ask questions, address concerns and make informed decisions together.” In contrast, referring patients to gynecology/adolescent medicine as a separate step can create barriers to care and increase the likelihood of delayed evaluation or loss to follow-up.
There are some nonhormonal options to help control heavy menstrual bleeding, including tranexamic acid, which may be preferred by select patients.
The physicians emphasize that early evaluation offers significant benefits with little downside. Identifying a bleeding disorder can help prevent complications throughout adolescence and adulthood, while patients without a bleeding disorder can benefit from earlier diagnosis and treatment of heavy menstrual bleeding, iron deficiency anemia and related issues.
For patients with a known bleeding disorder who have not yet begun menstruating, proactive counseling before menarche can be especially valuable. Discussing what to expect, recognizing when bleeding is abnormal and developing a management plan in advance can help patients and families feel more prepared.
“That's my goal as a gynecologist,” says Dr. Isaacson. “How do we get them back to living a normal life? Or how do we prevent emergency department visits and keep their period from becoming the center of everything?”
